ホテルコルディア大阪では、18歳未満もしくは高校生以下の未成年のご宿泊者様に宿泊同意書の提出をお願いしております📄🖋

ご予約の際、「未成年での宿泊」とお申し付けください。
宿泊同意書は保護者様にご記入頂きチェックインの際にフロントスタッフへお渡し、または事前にホテル宛へFAXをお送りください。

ご記入いただきました個人情報は本同意書の趣旨に限定して利用させていただきます。

未成年のご宿泊者全員分が必要となります。
チェックインの際に保護者様の同意が確認できない場合は下記のいずれかで宿泊同意の確認をいたします。

①：チェックインの際、その場で保護者様へ電話連絡による確認
②：または、後日宿泊同意書をホテル宛へ郵送
